Public Records
The Delaware Public Health District adheres to all Ohio Public Records Laws.
In accordance with the Ohio Revised Code section 149.43, Delaware Public Health District is required to make records available for public inspection within a reasonable time period. Additionally, in accordance with HIPAA, no personally medically identifiable information will be released via records requests.
Public Records Requests
You are not required to complete the request form to receive public records. Questions about public records and the retention schedule can be directed to gguillozet@delawarehealth.org, or 740-368-1700.
