Delaware, OH. – The Delaware Public Health District is excited to announce its new Care Coordination Program, along with the arrival of its first ever Community Health Worker, Krista Allen. The Care Coordination Program is a service to help individuals find a path to better health through the assistance of a Community Health Worker.

“I’m excited to have an opportunity to help our clients navigate resources throughout Delaware County.” Shares Allen. “I look forward to building a relationship with our partnerships and clients.”

Community Health Workers are trained and employed by Care Coordination agencies. When you work with a Pathway Hub agency such as the Delaware Public Health District, a Community Health Worker is assigned to you to support you or your family, where they will help you meet your health, social and economic needs.

Jen Keagy, Deputy Health Commissioner at DPHD, is just as thrilled as Allen is to be announcing the Care Coordination Program.

Keagy shares, “The Care Coordination Program will allow us to strengthen relationships with our partners, avoid duplication of services, and address the social determinants of health. Our CHW will walk with individuals and families along their journey to navigating their healthcare needs, with the goal of improving health outcomes and their quality of life.”
