The Vital Statistics office prepares paternity affidavits, death and birth certificate affidavits, burial permits as well as registering all death and birth events occurring in Delaware county.

Home Births are registered by the Registrar of the local health district.  Guidance on preparation of the appropriate forms is available by calling the Vital Statistics office.  The forms may be downloaded and completed, brought to the vital statistics office where filing will be completed.  Births should be registered within 10 days of birth.  For births registered 11 days or after a completed Delayed Birth Affidavit is required before filing can be completed.

Our staff can assist with other inquiries involving passport issues, name changes and identify fraud.

We also publish a newsletter for households who have newborns. The newsletter contains information about services available in Delaware County and the Delaware General Health District including immunizations and WIC programs.

The Registrar’s office at the Delaware General Health District is a participant in the Statewide Issuance Project. We can issue a birth certificate to anyone born in any county in Ohio.

Information necessary when making a request includes:

  • Name at death on the certificate
  • Date of death on the certificate
  • Name at birth on the certificate
  • Date of birth on the certificate
  • Maiden name of the mother (birth certificate)

Omission of any of the above information may result in a delay or inability to fill the request.

Walk-In Orders

The Vital Statistics office is located on the second floor of 1 West Winter Street. Walk-in hours are 8 a.m. to 4:30 p.m. Your certificate is prepared while you wait.

Mail Orders

Online Orders

Orders can be placed online using the VitalChek website.  VitalChek adds a service fee for processing this order and sends it to us to prepare.  Expedited shipping is offered.

The Vital Statistics Office will register death certificates presented in person or through the mail. We require a completed Funeral Home Application for Death Certificates to issue certified death certificates and burial permits.

Certificates to be registered should be mailed with the application form to the address listed on the side in an unfolded envelope. Please include a self addressed stamped envelope.

Burial Permits
Permits may be obtained from the Delaware General Health District Monday through Friday from 8:00 am to 4:30 pm. Additionally, three Sub Registrars in the Delaware General Health District are empowered to issue burial permits in the health district.
